Social Work Field Confirmation Form

This form gives the student and site the opportunity to confirm the placement and identify the mentor (i.e., site field supervisor).

To import this form into your environment, follow these steps:

• Download the xml file below
• Open your Forms module in the Sonia client
• Click the Import Template button
• Select this xml file from your computer
• Rename if necessary
• Save
