The Learning Contract form below is a sample that can be edited to meet the needs of your social work program.

Two templates are available for download: one reflects the 2015 EPAS, and the other reflects the 2022 EPAS.

To import this form into your environment, follow these steps:

• Download the xml file below
• Open your Forms module in the Sonia client
• Click the Import Template button
• Select this xml file from your computer
• Rename the form if necessary
• Save
