Student Assessment of Mentor and Placement

This form can be assigned at the end of the semester for the student to provide feedback about their placement site and mentor.

To import this form into your environment, follow these steps:

• Download the xml file below
• Open your Forms module in the Sonia client
• Click the Import Template button
• Select this xml file from your computer
• Rename if necessary
• Save
