Home-Based Placement Activity Form

This form would be used for students to confirm that they can successfully transition to a home-based placement. It covers the Requirements and Adjustments as well as Health and Safety for home-based health and safety working conditions.

As a reminder, to import this form into your environment, follow these steps:
1. Download the xml file below
2. Open your Forms module in the Sonia client
3. Click the Import Template button
4. Select this xml file from your computer
5. Rename if necessary
6. Save
