This form can be used to update site information.
Suggested process:
1. Put all active sites in a placement group as placement requests, and call it "Site Updates [year]"
2. Assign this form within the placement group to the primary contact of all of the sites
The above process is suggested so that you can track completion of the form and easily communicate with sites that do not complete the form.

To import this form into your environment, follow these steps:
1. Download the xml file below
2. Open your Forms module in the Sonia client
3. Click the Import Template button
4. Select this xml file from your computer
5. Rename if necessary
6. Save
