Evidence of Placement Form

This form would be used for students that may be asked to provide evidence to support travel between placement and campus or home (due to current travel restrictions). This Form has been designed for Students to access and print via Sonia Online, then carry with them.

The form is not editable and autofills with all necessary information. The Form references specific allocations and must be added to each allocation, this way Students will only have access to information for placements they are actually allocated to.

Placement Staff: Assign to allocated students as appropriate so students can view in Sonia Online.
Students: Access the Forms tab in Sonia Online, select to Edit the form, then Print so they can carry with them.

As a reminder, to import this form into your environment, follow these steps:
1. Download the xml file below
2. Open your Forms module in the Sonia client
3. Click the Import Template button
4. Select this xml file from your computer
5. Rename if necessary
6. Save
