Request to Complete Remote Field Hours

Students have a right to safety within their field education experience, and it is our hope that we can assist students in being successful in their field placement while also meeting accreditation standards.

As a reminder, to import this form into your environment, follow these steps:
1. Download the xml file below
2. Open your Forms module in the Sonia client
3. Click the Import Template button
4. Select this xml file from your computer
5. Rename if necessary
6. Save
