Placement Student Risk Assessment Form

Once the placement is confirmed as safe, then the students can complete this form on Sonia Online.

As a reminder, to import this form into your environment, follow these steps:
1. Download the xml file below
2. Open your Forms module in the Sonia client
3. Click the Import Template button
4. Select this xml file from your computer
5. Rename if necessary
6. Save
