Student Screening Tool

This form would be used to assess a student's potential exposure to COVID-19 to confirm they are safe to attend their placement.

As a reminder, to import this form into your environment, follow these steps:
1. Download the xml file below
2. Open your Forms module in the Sonia client
3. Click the Import Template button
4. Select this xml file from your computer
5. Rename if necessary
6. Save
