Placement Site Risk Assessment Form

This form should be used to identify where a placement can be reintegrated. The Placement Officer could either send the form to the site to complete or call the site and complete this form as part of a conversation around placement safety and suitability.

As a reminder, to import this form into your environment, follow these steps:
1. Download the xml file below
2. Open your Forms module in the Sonia client
3. Click the Import Template button
4. Select this xml file from your computer
5. Rename if necessary
6. Save
