Social Work Learning Agreement and Evaluations by Competency

This Social Work Learning Agreement and Evaluation form includes the Learning Agreement (where students can select activities) and fields for two evaluations.  This is a sample that can be edited to meet the needs of your social work program.

This version reflects the 2015 EPAS.

To import this form into your environment, follow these steps:

• Rename the form if necessary
