Social Work Learning Contract - By Competency

The Learning Contract form attached is a sample that can be edited to meet the needs of your social work program. It reflects the 2015 EPAS.

To import this form into your environment, follow these steps:

• Download the xml file below
• Open your Forms module in the Sonia client
• Click the Import Template button
• Select this xml file from your computer
• Rename the form if necessary
• Save
